All forms required by the Office of Student Financial Assistance (OSFA) may be downloaded as PDF files. Once downloaded, the forms should be completed, signed and mailed to OSFA at: 6525 N. Sheridan Rd., Chicago, IL 60626. Forms may also be faxed to: 773.508.3397 or scanned and emailed to lufinaid@luc.edu.
